Chemistry Panel Changes

 

Only two years ago, at HCFA’s urging, the American Medical Association’s CPT editorial board abolished the automated chemistry profile codes 80002-80019 and replaced them with four smaller disease/organ panels containing tests on Medicare’s national roster of automated procedures.  HCFA has steadily tightened requirements for panels, in particular stipulating that for a panel to be reimbursed, all of its component tests must be medically justified. 

 

Not surprisingly, the CPT 2000 manual once again contained significant changes to the organ/disease panels that laboratories must implement on April 1.  These changes directly affect the way laboratories bill Medicare and other federal and state health care programs for these panels and may affect the amount of Medicare reimbursement received for each panel.

The most notable chemistry panel changes are summarized below:

 

·         A new renal function panel was created.  The CPT code is 80069.

·         ALT was added to the comprehensive metabolic panel. The CPT code changed from 80054 to 80053.

·         Calcium was added to the basic metabolic panel.  The CPT code changed from 80049 to 80048. 

·         Total protein was added to the hepatic function panel. The CPT code changed from 80058 to 80076.  


 

In addition to these chemistry panel changes, three other notable changes occurred.

 

·         A new four-test acute hepatitis panel (80074) replaced the current hepatitis panel (CPT 80059). 
Hepatitis A IgM antibody replaced hepatitis A IgG and IgM antibodies and hepatitis B core IgM antibody replaced hepatitis B core IgG and IgM antibodies.  The new panel is actually the panel that most laboratories have offered for many years to diagnose acute viral hepatitis. 

·         Thyroid panel (80091), which consisted of total T4 and T3 uptake, was deleted.

·         Thyroid panel (80092), which consisted of total T4, T3 uptake, and TSH, was deleted.
